Insurance Procedure
All deductibles and co-payments are the patient’s financial responsibility and are required at the time of your visit. Please bring your insurance card(s) to your appointments. We will bill your insurance for any insurance plan for which we are a participating provider, but final payment is the patient’s responsibility. Your insurance coverage is a contract between you and your insurance company; we do not determine the specifics of your benefits.
Medicare
You are financially responsible for any unmet deductible, as well as for any portion not covered by your secondary insurance. As Medicare providers, we will file your claim for you, and accept payment directly from Medicare.
